New Application: To begin a new application, click "new user" below. If you have previously held a NC license or have ever completed an application, you are not considered a "New User". You will need to recover your file ID by clicking the link at the bottom of the page.

Important: During this application process you will be required to download forms using Acrobat Reader. If you do not have Acrobat Reader, click here to download.

Please be aware of the following:

1. You must complete the online portion of the application and pay within 30 days or your application will be deleted. You have one year from the date of payment to complete the entire application process. You will be notified of the expiration date each time you log on and access your application while it is pending. If your application is not complete within this timeframe, it will expire and you will no longer have access to that application. If you wish to continue seeking licensure, you will need to start a new application and pay a new application and background check fee.
2. Please note that this application is for Physicians that wish to obtain a Graduate Medical Training License. This is not a registration renewal.
3. Please turn off your pop-up blocker in order to complete this online application.
4. PRINTER ACCESS NEEDED TO COMPLETE THE ONLINE APPLICATION. You must print out various forms that support your application before you pay for the application. After you enter your valid credit card (Visa/Mastercard/Discover/American Express) you must print the application receipt. Payment by check or money order is not acceptable and will be returned. Processing of the application will not begin until payment has been received.
5. The online status will not be updated until the fee has been paid. You may give your log-in information (File ID and DOB) to anyone you want to check your online status during the application process.
6. If you have previously applied for any type of North Carolina medical license, some of your information will automatically be pre-populated into this application.
7. Attention: This is important. Making false statements or representations to the Board, or willfully concealing from the Board material information in connection with your application for a license may result in denial of your application or other disciplinary action. All license denials are reported to the National Practitioners Data Bank (NPDB) and other state medical boards. If you are in doubt about whether to report any information requested you are advised to fully disclose the information and provide an explanation of the circumstances.
